Homeownership Initiative Forms (HOP and NIP)
All forms for HOP and NIP are in Microsoft Word 2000, Microsoft Excel, or
PDF (Adobe Acrobat) format. Click the link to open the file. Print, tab through
the blank fields to enter information and then print, or save the file to your
local PC drive. Additional tools are available to help administer our programs.
Participation
To participate in any Homeownership Initiatives Program (HOP and NIP)
member financial institutions of the FHLBI must submit a registration
form and agreement. Upon approval, a starter kit will be sent with
information needed to start requesting disbursements. Note: Members who
completed this process in 2007 are not required to re-enroll and participate
in web-based training in 2008.
2008 HOPReserve
Applications are
updated annually, are due late February, and awards are announced early
March. If you've missed the deadline, please plan to download a new
application next year.
HUD AMI charts
AMI is automatically calculated on the disbursement request forms (via Excel formulas).
These HUD AMI charts are also available as a reference.
Income Calculation Form
To provide information regarding what documentation and calculations were used in performing the household income qualification.
Returning funds
Retention agreement
Each homebuyer must complete this legal document before disbursement will be made.
The form is an agreement to stay in the home for a certain period of time.
Rehabilitation closing statement
A settlement or closing statement must be provided to the FHLBI for each rehabilitation grant.
