AHP Forms
All of the forms below are in Microsoft Word 2002, PDF format (use Adobe Acrobat Reader), or Microsoft Excel 2002.
Just click to open and save to your local PC drive or print. All forms are form-enabled to allow users
to enter data directly onto the forms.
Documentation required to support the form will be specified in the instructions. Many of the forms are specific to
rental or owner-occupied projects only.
> Application for AHP award
Income
AMI is automatically calculated on the disbursement request forms (via Excel formulas). These HUD AMI charts are
also available as a reference.

Retention
After winning an AHP award, submit a notarized retention agreement per instructions in the AHP Implementation
Plan.

Disbursement
To draw funds for AHP, submit a disbursement request form and a disbursement checklist, as well as any other material
specified on the checklist.

Monitoring during project
Each AHP user will be sent a partially completed semi-annual progress form at the proper time. If an extra form is
needed, it is available here.
